About Us

Rocky Mountain PACE (Program of All-Inclusive Care for the Elderly) is a managed health care plan exclusively for older adults. RM PACE participants receive primary medical care, medication management, physical therapy, specialty services, in-home care, social work assistance, and transportation to and from the RM PACE day center and medical appointments. With RM PACE, you can remain living in the comfort of your home and get the medical care you need in a safe environment.
PACE is a health plan designed exclusively for seniors. If you are age 55 and older, dealing with multiple health challenges and need assistance in your daily routine, Rocky Mountain PACE might be just the help you need.
You and your care team will work together to create a personalized care plan that may include some or all of the services provided by Rocky Mountain PACE. These depend on your medical needs, which may change over time. Your care is provided in your home, at our centers, or via telehealth.
Your PACE care team includes 11 professional disciplines, and every member of your care team understands your needs.
Each week, this team meets to review care plans and changes in medical conditions, social engagement, and emotional well-being.
We have 3 Centers in Colorado Springs. This is where we'll work with you on your strength,and balance in the senior gym, introduce you to new activities, eat nutritious meals, and make new friends.
